If you are a client, please sign in using the user-id and password you were assigned. Heres what to do:
Login to the Patient Login section and you will be taken to the HOME screen where you will see a MY TO DO LIST. Please click on each link under the heading MESSAGE and then read and complete each form we have initially placed in your MY TO DO LIST. Once you have read the required form scroll down to the bottom of the page and enter your password and click SIGN AND COMPLETE DOCUMENT.
The information you submit is secured and no one else has access to your clinical information as it is password protected .Once logged in, you will have the option of changing your password by clicking on change password on the left top portion of the screen. In order to get back to the main page to continue with the To Do List, click the back button at the bottom of each form. For clients under the age of 18, a parent or guardian will need to fill out each form for the client and electronically sign each form.

FROM PROVIDENCE:

- Take Route 195 East
- Drive 1.4 miles past the George Washington Bridge. Take Exit 7 onto Route 114 South towards Seekonk/Barrington. Remain in the far right lane.
- Approximately 1.1 miles after taking this exit and shortly before an overpass, you will come to the first exit on your right Pawtucket Ave./114 North take this exit.
- At the end of the ramp, turn right. Immediately after making the right turn, there will be a 3-story red brick office building on your right. This is the East Bay Medical Centre. I am in Suite 303.
FROM BARRINGTON AND POINTS FURTHER SOUTH IN THE EAST BAY:

- Follow County Road through Barrington until it merges with Route 114 North.
- Continue on Route 114 until you pass the Gate of Heaven Cemetery on your right. Take the very next exit for Pawtucket Ave.
- Drive about one-third of a mile. You will pass over Route 114 and will see a 3-story red brick office building on your right. This is East Bay Medical Centre. I am in Suite 303.
FROM SEEKONK/SWANSEA OR POINTS FURTHER EAST:

- Take I-195 West towards Providence. Take Exit 1 in RI for Route 114A towards Seekonk/Barrington
- At the end of the exit take a left at the stop light onto Fall River Ave.
- Follow this road until it ends at an intersection (there will be a gas station on your right and a hotel in front of you). Get into the right hand lane at this intersection.
- Just before the next stop light, bear right onto Mink St. Follow this for approximately 0.4 miles.
- There will be a Mobil gas station on your right. Bear right onto Route 114 North.
- Continue briefly on Route 114 until you pass the Gate of Heaven Cemetery on your right. Take the next exit for Pawtucket Ave.
